Survey Deficiencies

If a surveyor has cited your facility for pest activity or pesticide handling, the citation names a tag or an Element of Performance. These pages take each identifier in turn: what it enforces, what the surveyor was looking for, the documentation that closes the finding, where facilities usually fail, and the elements a plan of correction has to address. If you do not know which regime's identifier applies to your facility, or the number on your paperwork is not listed here, start at Which tag is pest control?

These pages are reference, not templates. A plan of correction is a facility's own document and is reviewed by the agency that issued the finding; nothing here should be submitted without review by the people accountable for it. Pages whose standard text is paywalled or not yet transcribed from the primary source say so in the body rather than paraphrasing it.

A-0700 Condition of Participation: Physical Environment
The condition-level citation for 42 CFR §482.41 as a whole. Used when a physical environment deficiency, including a pest condition, is judged systemic or serious enough to place the whole condition out of compliance. Verified in Rev. 238.
A-0701 Standard: Buildings
The tag most pest observations in patient-care, storage, and support areas are written to. Enforces §482.41(a): the physical plant must be maintained so that patient safety and well-being are assured. Verified in Rev. 238.
A-0722 Standard: Facilities (§482.41(d)), with A-0724
The standard-level tag for §482.41(d), the hospital must maintain adequate facilities for its services, and its sub-tag A-0724 (§482.41(d)(2)), whose guidance on the condition of the physical plant and the safe storage of supplies is the closest hospital-side hook for a pest finding in food service and support areas. A-0747 Condition of Participation: Infection Prevention and Control and Antibiotic Stewardship Programs
The condition-level tag for §482.42. Cross-referenced when pest activity is framed as an infection control failure rather than, or in addition to, a physical environment failure. Verified in Rev. 238.
A-0749 Infection Prevention and Control Program: Methods for Preventing and Controlling Transmission (§482.42(a)(2))
The tag for §482.42(a)(2): the infection prevention and control program must employ methods for preventing and controlling transmission of infections within the hospital and between settings. Verified in Rev. 238. Carries no pest language; a pest finding framed as infection control is far more likely to be written at A-0750.
A-0750 Clean and Sanitary Environment (§482.42(a)(3))
The tag that carries the manual's sanitary-environment language: the program must include maintaining a clean and sanitary environment, and the guidance lists the areas to monitor (food storage, dish rooms, ice machines, waste handling, surgical areas, supply storage). Pest control is not named; this is where a pest finding framed as infection control is most likely written.
A-0758 A-0758 does not appear in CMS Appendix A
A-0758 is not a tag in Appendix A Rev. 238; the sequence runs A-0750 → A-0751 → A-0760, and A-0752–A-0759 do not exist. F925 Effective Pest Control Program (§483.90(i)(4))
The only explicit pest control mandate in the CMS long-term care requirements: the facility must maintain an effective pest control program so that it is free of pests and rodents. CMS defines the program and names the pests. Verbatim from Appendix PP Rev. 232.
