The Correction

There is no tag A-0758 in the CMS State Operations Manual, Appendix A. In the current edition, Rev. 238 (Issued 03-20-26), every tag heading in the A-0740 to A-0799 range, in document order, is:

A-0747, A-0748, A-0749, A-0750, A-0751, A-0760, A-0761, A-0762, A-0763, A-0764, A-0765, A-0770, A-0771, A-0772, A-0773, A-0774, A-0775, A-0776, A-0777, A-0778, A-0779, A-0780, A-0781, A-0785, A-0786, A-0787, A-0788, A-0789, A-0799

A-0751 is followed directly by A-0760. A-0752 through A-0759 do not exist. Nothing exists between A-0730 and A-0747 either.

Appendix A also contains no pest guidance under any tag. Measured on the full 613-page text of Rev. 238 (case-insensitive; “substring” catches plurals and compounds such as “pests,” “rodents,” “insecticide,” “infestations”):

The counts are what make this page citable: a hospital cannot be cited under an “A-0758 pest tag” because there is no A-0758 and there is no pest tag. Both facts were measured against the document itself on September 11, 2026 and agree with the operator’s independent search of the same PDF on September 10, 2026.

Hospital pest conditions are cited as failures of the physical plant, under A-0701 (42 CFR §482.41(a)) or A-0724 (§482.41(d)(2)), or of the sanitary environment, under A-0750 (§482.42(a)(3)); in dietary areas the hospital’s kitchen sanitation obligations under §482.28 (tag A-0618 and following) are also in play. If you arrived here with “A-0758” written on a citation, a checklist, a vendor report, or a consultant’s template, the number is wrong. The Two Tags on Either Side of the Gap

So that a reader holding a mistyped citation can tell which neighbor it was meant to be, the headings verbatim from Rev. 238:

A-0751 §482.42(a)(4) The infection prevention and control program reflects the scope and complexity of the hospital services provided.

A-0760 §482.42(b)(1) Standard: Antibiotic stewardship program organization and policies. The hospital must demonstrate that: (1) An individual (or individuals), who is qualified through education, training, or experience in infectious diseases and/or antibiotic stewardship, is appointed by the governing body as the leader(s) of the antibiotic stewardship program and that the appointment is based on the recommendations of medical staff leadership and pharmacy leadership;

Neither carries pest guidance; nothing in the manual does.

A-0701 — Standard: Buildings (42 CFR §482.41(a))

The tag heading, verbatim from Rev. 238: “§482.41(a) Standard: Buildings”. The regulation text printed beneath it, identical to eCFR:

(a) Standard: Buildings. The condition of the physical plant and the overall hospital environment must be developed and maintained in such a manner that the safety and well-being of patients are assured.

The regulation contains no pest language. Pest activity in a patient-care area, a storage room, or a support space is cited here as a failure to maintain the environment so that patient safety and well-being are assured. Full treatment: A-0701.

A-0724 — Facilities, Supplies, and Equipment (42 CFR §482.41(d)(2))

The tag heading, verbatim: “§482.41(d)(2) - Facilities, supplies, and equipment must be maintained to ensure an acceptable level of safety and quality.” The Interpretive Guidelines open:

The hospital must ensure that the condition of the physical plant and overall hospital environment is developed and maintained in a manner that provides an acceptable level of safety and well-being of patients, staff and visitors.

That sentence is the closest hospital-side hook for a pest finding under the Physical Environment condition; the parent standard is covered at A-0722.

A-0750 — Sanitary Environment (42 CFR §482.42(a)(3))

The tag heading, verbatim, is the regulation text:

§482.42(a)(3) The infection prevention and control program includes surveillance, prevention, and control of HAIs, including maintaining a clean and sanitary environment to avoid sources and transmission of infection, and addresses any infection control issues identified by public health authorities; and

The Interpretive Guidelines open:

The hospital must provide and maintain a clean and sanitary environment to avoid sources and transmission of infections and communicable diseases. All areas of the hospital must be clean and sanitary. This includes all hospital departments and off-site locations.

and go on to list the areas the program is expected to monitor, including “food storage, preparation, serving and dish rooms, refrigerators, ice machines, air handlers, … waste handling, surgical areas, supply storage.” Pest control is not named. A pest finding cited to A-0750 is a finding that an area is not clean and sanitary, or that the program’s monitoring did not catch a source of infection, with pest activity as the source. Correction to this reference’s earlier text. Until September 11, 2026, this page and three others quoted a sentence beginning “The hospital must provide a sanitary environment to avoid sources and transmission of infections and communicable diseases. There must be an active program…” as the Appendix A sanitary-environment language. That sentence does not occur in Rev. 238; the passage above is what the manual actually says. The earlier wording tracks the pre-2019 text of §482.42. It has been replaced everywhere it appeared.

How to Read the Tag on Your Form CMS-2567

Appendix A’s survey protocol describes the Statement of Deficiencies this way:

The Form CMS-2567 is made public no later than 90 calendar days following completion of the survey. It documents specific deficiencies cited, the facility’s plans for correction and timeframes, and it provides an opportunity for the facility to refute survey findings and furnish documentation that requirements are met.

and states the deadline that starts when you receive it:

Inform the facility that a written plan of correction must be submitted to the survey agency within 10 calendar days following receipt of the written statement of deficiencies.

On the form itself, the left side of each page is the agency’s and the right side is the hospital’s. The tag column on the left carries the letter prefix (A for hospitals) and the four-digit number, printed as “A 0701” or “A0701.” Beneath the tag the form prints the regulation text the tag enforces, then the surveyor’s summary of what was observed, where, and when. The right side, headed as the provider’s plan of correction, is completed by the hospital opposite each tag, with a completion date for each corrective action. Read the regulation text printed beneath the tag before anything else. It is the regulation, not the tag number, that the plan of correction has to answer; the tag is the index.

Why the Number Is Wrong Is Not Known

This reference does not know how “A-0758” entered circulation and does not speculate. Search logs show it being queried as “cms som appendix a tag a-0758” and “cms hospital survey tag a-0758”; this page exists so that the search lands on the record rather than on a repetition of the error.

What This Page Does Not Say

• It does not say CMS has no authority over pest conditions in hospitals. It does, by inference, under the tags above.
• It does not say a citation written to another tag is invalid. A surveyor may cite any tag whose regulation text the observation supports; the regulation text on the form controls.
• It does not say the counts hold for any revision other than Rev. 238 (Issued 03-20-26). CMS reissues Appendix A several times a year; the measurement is tied to the named revision.
